It leads us to meditate upon those factors which, in our day, should spiritually unite rather than divide-Jews, Christians and Muslims.
As a surgeon, Maurice Bucaille has often been in a situation where he was able to examine not only people's bodies, but their souls. This is how he was struck by the existence of Muslim piety and by aspects of Islam which remain unknown to the vast majority of non-Muslims. In his search for explanations which are otherwise difficult to obtain, he learnt Arabic and studied the Qur'an. In it, he was surprised to find statements on natural phenomena whose meaning can only be understood through modern scientific knowledge.
He then turned to the question of the authenticity of the writings that constitute the Holy Scriptures of the monotheistic religions. Finally, in the case of the Bible, he proceeded to a confrontation between these writings and scientific data.
The results of his research into the Judeo-Christian Revelation and the Qur'an are set out in this book.

Dalam organisme yang berkembang biak secara seksual, ketika sebuah sperma membuahi sebuah ovum, hasilnya adalah sebuah sel yang disebut zygot yang memiliki seluruh DNA dari kedua orang tuanya. Dalam tumbuhan, hewa, dan beberapa protista, zygot akan mulai membelah oleh mitosis untuk menghasilkan organisme multisel. Hasil dari proses ini disebut embrio.
Dalam hewan, perkembangan zygot menjadi embrio terjadi melalui tahapan yang dikenal sebagai blastula, gastrula, dan organogenesis.
Janin

Janin (fetus) pada usia delapan minggu.
Janin atau fetus, adalah mamalia yang berkembang setelah fase embrio dan sebelum kelahiran. Dalam bahasa Latin, fetus secara harfiah dapat diartikan "berisi bibit muda, mengandung". Spermatogenesis
Peralihan dari bakal sel kelamin yang aktif membelah ke sperma yang masak serta menyangkut berbagai macam perubahan struktur yang berlangsung secara berurutan. Spermatogenesis berlangsung pada tubulus seminiferus dan diatur oleh hormone gonadtotropin dan testosterone (Wildan yatim, 1990).
Tahap pembentukan spermatozoa dibagi atas tiga tahap yaitu :
1.Spermatocytogenesis
Merupakan spermatogonia yang mengalami mitosis berkali-kali yang akan menjadi spermatosit primer.
Spermatogonia
Spermatogonia merupakan struktur primitif dan dapat melakukan reproduksi (membelah) dengan cara mitosis. Spermatogonia ini mendapatkan nutrisi dari sel-sel sertoli dan berkembang menjadi spermatosit primer.
Spermatosit Primer
Spermatosit primer mengandung kromosom diploid (2n) pada inti selnya dan mengalami meiosis. Satu spermatosit akan menghasilkan dua sel anak, yaitu spermatosit sekunder.
2. Tahapan Meiois
Spermatosit I (primer) menjauh dari lamina basalis, sitoplasma makin banyak dan segera mengalami meiosis I yang kemudian diikuti dengan meiosis II.
Sitokenesis pada meiosis I dan II ternyata tidak membagi sel benih yang lengkap terpisah, tapi masih berhubungan sesame lewat suatu jembatan (Interceluler bridge). Dibandingkan dengan spermatosit I, spermatosit II memiliki inti yang gelap.
3. Tahapan Spermiogenesis
1. Merupakan transformasi spermatid menjadi spermatozoa yang meliputi 4 fase yaitu fase golgi, fase tutup, fase akrosom dan fase pematangan. Hasil akhir berupa empat spermatozoa masak. Dua spermatozoa akan membawa kromosom penentu jenis kelamin wanita “X”. Apabila salah satu dari spermatozoa ini bersatu dengan ovum, maka pola sel somatik manusia yang 23 pasang kromosom itu akan dipertahankan.
Oogenesis
Sel-Sel Kelamin Primordial
Sel-sel kelamin primordial mula-mula terlihat di dalam ektoderm embrional dari saccus vitellinus, dan mengadakan migrasi ke epitelium germinativum kira-kira pada minggu ke 6 kehidupan intrauteri. Masing-masing sel kelamin primordial (oogonium) dikelilingi oleh sel-sel pregranulosa yang melindungi dan memberi nutrien oogonium dan secara bersama-sama membentuk folikel primordial.

Folikel PrimordiaL
Folikel primordial mengadakan migrasi ke stroma cortex ovarium dan folikel ini dihasilkan sebanyak 200.000. Sejumlah folikel primordial berupaya berkembang selama kehidupan intrauteri dan selama masa kanak-kanak, tetapi tidak satupun mencapai pemasakan. Pada waktu pubertas satu folikel dapat menyelesaikan proses pemasakan dan disebut folikel de Graaf dimana didalamnya terdapat sel kelamin yang disebut oosit primer.
Oosit Primer
Inti (nukleus) oosit primer mengandung 23 pasang kromosom (2n). Satu pasang kromosom merupakan kromosom yang menentukan jenis kelamin, dan disebut kromosom XX. Kromosom-kromosom yang lain disebut autosom. Satu kromosom terdiri dari dua kromatin. Kromatin membawa gen-gen yang disebut DNA.

Perkembangan Janin di Rahim
Pembelahan
Menurut yatim (1990:155) pada manusia pembelahan terjadi secara holobastik tidak teratur. Dimana bidang dan waktu tahap-tahap pembelahan tidak sama dan tidak serentak pada berbagai daerah zigot. Awalnya zigot membelah menjadi 2 sel, kemudian terjadi tingkat 3 sel, kemudian tingkat 4 sel, diteruskan tingkat 5 sel, 6 sel, 7 sel, 8 sel, dan terus menerus hingga terbentuk balstomer yang terdiri dari 60-70 sel, berupa gumpalan massif yang disebut morula.
Pembelahan atau segmentasi terjadi setelah pembelahan. Zigot membelah berulang kali sampai terdiri dari berpuluh sel kecil yang disebut blastomer. Pembelahan itu bias meliputi seluruh bagian, bias pula hanya sebagian kecil zigot. Pembelahan ini terjadi secara mitosis. Bidang yang ditempuh oleh arah pembelahan ketika zigot mengalami mitosis terus-menerus menjadi banyak sel, disebut bidang pembelahan. Ada 4 macam bidang pembelahan yaitu meridian, vertical, ekuator dan latitudinal

Blastulasi dan Nidasi
Setelah sel-sel morula mengalami pembelahan terus-menerus maka akan terbentuk rongga di tengah. Rongga ini makin lama makin besar dan berisi cairan. Embrio yang memiliki rongga disebut blastula, rongganya disebut blastocoel, proses pembentukan blastula disebut blastulasi.
Pembelahan hingga terbentuk blastula ini terjadi di oviduk dan berlangsung selama 5 hari. Selanjutnya blastula akan mengalir ke dalam uterus. Setelah memasuki uterus, mula-mula blastosis terapung-apung di dalam lumen uteus. Kemudian, 6-7 hari setelah fertilisasi embryo akan mengadakan pertautan dengan dinding uterus untuk dapat berkembang ke tahap selanjutnya. Peristiwa terpautnya antara embryo pada endometrium uterus disebut implantasi atau nidasi. Implantasi ini telah lengkap pada 12 hari setelah fertilisasi (Yatim, 1990: 136)
Gastrulasi
Menurut Tenzer (2000:212) Setelah tahap blastula selesai dilanjutkan dengan tahap gastrulasi. Gastrula berlangsung pada hari ke 15. Tahap gastrula ini merupakan tahap atau stadium paling kritis bagi embryo. Pada gastrulasi terjadi perkembangan embryo yang dinamis karena terjadi perpindahan sel, perubahan bentuk sel dan pengorganisasian embryo dalam suatu sistem sumbu. Kumpulan sel yang semula terletak berjauhan, sekarang terletak cukup dekat untuk melakukan interkasi yang bersifat merangsang dalam pembentukan sistem organ-organ tbuh. Gastrulasi ini menghasilkan 3 lapisan lembaga yaitu laisan endoderm di sebelah dalam, mesoderm disebelah tengah dan ectoderm di sebelah luar.
Dalam proses gastrulasi disamping terus menerus terjadi pembelahan dan perbanyakan sel, terjadi pula berbagai macam gerakan sel di dalam usaha mengatur dan menyusun sesuai dengan bentuk dan susunan tubuh individu dari spesies yang bersangkutan.
Tubulasi
Tubulasi adalah pertumbuhan yang mengiringi pembentukan gastrula atau disebut juga dengan pembumbungan. Daerah-daerah bakal pembentuk alat atau ketiga lapis benih ectoderm, mesoderm dan endoderm, menyusun diri sehingga berupa bumbung, berongga. Yang tidak mengalami pembumbungan yaitu notochord, tetapi masif. Mengiringi proses tubulasi terjadi proses differensiasi setempat pada tiap bumbung ketiga lapis benih, yang pada pertumbuhan berikutnya akan menumbuhkan alat (organ) bentuk definitif. Ketika tubulasi ectoderm saraf berlangsung, terjadi pula differensiasi awal pada daerah-daerah bumbung itu, bagian depan tubuh menjadi encephalon (otak) dan bagian belakang menjadi medulla spinalis bagi bumbung neural (saraf). Pada bumbung endoderm terjadi differensiasi awal saluran atas bagian depan, tengah dan belakang. Pada bumbung mesoderm terjadi differensiasi awal untuk menumbuhkan otot rangka, bagian dermis kulit dan jaringan pengikat lain, otot visera, rangka dan alat urogenitalia.
Tahap Perkembangan Fetus/Janin
Tahap perkembangan janin dimulai pada bulan ke 3 sampai ke 10.
Pada 6 bulan terakhir perkembangan manusia digunakan untuk meningkatkan ukuran dan mematangkan organ-organ yang dibentuk pada 3 bulan pertama.
Pada saat janin memasuki bulan ke 3, panjangnya 40 mm. Janin sudah mempunyai sistem organ seperti yang dipunyai oleh orang dewasa. Pada usia ini genitalnya belum dapat dibedakan antara jantan dan betina dan tampak seperti betina serta denyut jantung sudah dapat didengarkan.
Pada bulan ke 4 ukuran janin 56 mm. Kepala masih dominan dibandingkan bagian badan, genitalia eksternal nampak berbeda. Pada minggu ke 16 semua organ vital sudah terbentuk. Pembesaran uterus sudah dapat dirasakan oleh ibu.
Pada bulan ke 5 ukuran janin 112 mm, sedangkan akhir bulan ke 5 ukuran fetus mencapai 160 mm. Muka nampak seperti manusia dan rambut mulai nampak diseluruh tubuh (lanugo). Pada yang jantan testis mulai menempati tempat dimana ia akan turun ke dalam skrotum. Gerakan janin sudah dapat dirasakan oleh ibu. Paru-paru sudah selesai dibentuk tapi belum berfungsi.
Pada bulan ke 6 ukuran tubuh sudah lebih proporsional tapi nampak kurus, organ internal sudah pada posisi normal.
Pada bulan ke 7 janin nampak kurus, keriput dan berwarna merah. Skrotum berkembang dan testis mulai turun untuk masuk ke skrotum, hal ini selesai pada bulan ke 9. system saraf berkembang sehingga cukup untuk mengatur pergerakan fetus, jika dilahirkan 10% dapat bertahan hidup.
Pada bulan ke 8 testis ada dalam skrotum dan tubuh mulai ditumbuhi lemak sehingga terlihat halus dan berisi. Berat badan mulai naik jika dilahirkan 70% dapat bertahan hidup.
Pada bulan ke 9, janin lebih banyak tertutup lemak (vernix caseosa). Kuku mulai nampak pada ujung jari tangan dan kaki.
Pada bulan ke 10, tubuh janin semakin besar maka ruang gerak menjadi berkurang dan lanugo mulai menghilang. Percabangn paru lengkap tapi tidak berfungsi sampai lahir. Induk mensuplai antibodi plasenta mulai regresi dan pembuluh darah palsenta juga mulai regresi.
Proses Terbentuknya janin laki-laki dan perempuan
Proses terbentuknya janin laki-laki dan perempuan dimulai dari deferensiasai gonad. Awalnya sel sperma yang berkromosom Y akan berdeferensiasi awal menjadi organ jantan dan yang X menjadi organ betina. Deferensiasi lanjut kromosom Y membentuk testis sedangkan kromosom X membentuk ovarium. Proses deferensiasi menjadi testis dimulai dari degenerasi cortex dari gonad dan medulla gonad membentuk tubulus semineferus. Di celah tubulus sel mesenkim membentuk jaringan intertistial bersama sel leydig. Sel leydig bersama dengan sel sertoli membentuk testosteron dan duktus muller tp duktus muller berdegenerasi akibat adanya faktor anti duktus muller, testosteron berdeferensiasi menjadi epididimis, vas deferent, vesikula seminlis dan duktus mesonefros. Karena ada enzim 5 alfareduktase testosteron berdeferensiasi menjadi dihidrotestosteron yang kemudian pada epitel uretra terbentuk prostat dan bulbouretra. Selanjunya mengalami pembengkakan dan terbentuk skrotum. Kemudian testis turun ke pelvis terus menuju ke skrotum. Mula-mula testis berada di cekukan bakal skrotum saat skrotum mkin lmamakin besar testis terpisah dari rongga pelvis.
Sedangkan kromosom X yang telah mengalami deferensiasi lanjut kemudian pit primer berdegenerasi membentuk medula yang terisi mesenkim dan pembuluh darah, epitel germinal menebal membentuk sel folikel yang berkembang menjadi folikel telur. Deferensiasi gonad jadi ovarium terjadi setelah beberapa hari defrensiasi testis. Di sini cortex tumbuh membina ovarium sedangkan medula menciut. PGH dari placenta mendorong pertumbuhan sel induk menjadi oogonia, lalu berplorifrasi menjadi oosit primer. Pada perempuan duktus mesonefros degenerasi. Saat gonad yang berdeferensiasi menjadi ovarium turun smpai rongga pelvis kemudian berpusing sekitar 450 letaknya menjadi melintang.
Penis dan klitoris awalnya pertumbuhannya sama yaitu berupa invagina ectoderm. Klitoris sebenarnya merupakan sebuh penis yang tidak berkembang secara sempurna. Pada laki-laki evagina ectoderm berkembang bersama terbawanya sinus urogenitalis dari cloaca.

Hal itu dikemukakan Direktur Pemberantasan Penyakit Bersumber Binatang ( P2B2 ) Direktorat Pemberantasan Penyakit Menular dan Penyehatan Lingkungan ( P2M & PL ) Departemen Kesehatan, Dr Thomas Suroso, MPH ( Selasa 2 Februari 2003 ). Saat ini tim dari P2M&PL, Badan Penelitian dan Pengembangan Kesehatan ( Litbangkes ) serta unit penelitian Angkatan Laut AS ( NAMRU ) sedang ke Bolaang Mangondow, Sedangkan contoh darah dari penderita dari Kabupaten Bandung ( Cikalongwetan ) dan Jember akan diperiksa di laboratorium Litbangkes.
Sejauh ini di Kecamatan Cikalongwetan, Kabupaten Bandung ( Desa Mandalamukti ) tercatat ada 218 penderita, di Jember , demikian Kepala Sub-Direktorat Arbovirosis Dr. Rita Kusriastuti,Msc, ada 149 penderita ( Desa BalungLor ) dan kabupaten Bolaang Mangondow, 608 penderita. Chikungunya berasal dari bahasa Shawill yang berarti ? Yang Berubah Bentuk atau Bungkuk ? mengacu pada postur penderita yang membungkuk akibat nyeri sendi hebat ( Arthralgia ), Nyeri sendi ini menurut lembar data keselamatan ( MSDS ) Kantor Keamanan Laboratorium Kanada, terutama terjadi pada ? Lutut Pergelangan Kaki Serta Persendian Tangan Dan Kaki ?.
Gejala Demam Chikungunya mirip dengan Demam Berdarah Dengue yaitu Demam yang tinggi, menggigil, sakit kepala, mual, muntah, sakit perut, nyeri sendi dan otot serta bintik-bintik merah pada kulit terutama badan dan lengan. Bedanya dengan demam berdarah dengue, pada chikungunya tidak ada perdarahan hebat, renjatan ( Schok ) maupun kematian. Masa inkubasi dari demam Chikungunya dua sampai empat hari. Manifestasi penyakit berlangsung tiga sampai 10 hari . Virus ini termasuk ? Self Limiting Disease ? alias hilang dengan sendirinya. Namun rasa nyeri masih tertinggal dalam hitungan minggu sampai bulan. Tak ada vaksin maupun obat khusus untuk Chikungunya. ?Cukup minum obat penurun panas dan penghilang rasa sakit yang bisa dibeli di warung, yang penting cukup istirahat, minum dan makanan bergizi ?, saran Thomas.
Menurut situs Universitas Standford, virus Chikungunya masuk keluarga Togaviridae, genus alphavirus, dan ditularkan Nyamuk Aedes Aegypti. Virus ini terus menimbulkan epidemi di wilayah tropis Asia dan Afrika sejak diidentifikasi tahun 1952 di Afrika Timur. Di Indonesia Demam Chikungunya dilaporkan pertama kali di Samarinda tahun 1973. Kemudian berjangkit di Kuala Tunkal, Jambi, tahun 1980. Tahun 1983 merebak di martapura, ternate dan Yogyakarta. Setelah vakum hampir 20 tahun, awal tahun 2001 kejadian luar biasa ( KLB ) demam Chikungunya terjadi di Muara Enim, Sumatera Selatan dan Aceh. Disusul Bogor bulan Oktober. Demam Chikungunya berjangkit lagi di Bekasi ( Jawa Barat ), Purworejo dan Klaten ( Jawa Tengah ) tahun 2002. Dari literatur yang saya baca, memang ada gelombang epidemi 20 tahunan. Mungkin terkait perubahan iklim dan cuaca, ujar Thomas. Penjelasan lain, menurut situs Keamanan Laboratorium Kanada, antibodi yang timbul dari penyakit ini membuat penderita kebal terhadap serangan virus selanjutnya. Oleh karena itu, perlu waktu panjang bagi penyakit ini untuk merebak kembali. Menurut Thomas dan Rita, tak ada cara lain untuk mencegah Demam Chikungunya kecuali mencegah gigitan nyamuk serta memberantas tempat perindukan nyamuk dengan tiga M ( Menutup, Menguras dan Mengubur barang bekas yang bisa menampung air ) atau menaburkan bubuk abate pada penampungan air sebagaimana mencegah Demam Berdarah.
DEMAM CHIKUNGUNYA
Virus Chikungunya pertama kali diidentifikasi di Afrika Timur tahun 1952. Tidak heran bila namanya pun berasal dari bahasa Swahlii, Artinya adalah yang berubah bentuk atau bungkuk, Postur penderitanya memang kebanyakan membungkuk akibat nyeri hebat di persendian tangan dan kaki. Virus ini termasuk keluarga Togaviridae, Genus alphavirus, dan ditularkan oleh Nyamuk Aedes Aegypti. Gejalanya adalah demam tinggi, sakit perut, mual, muntah, sakit kepala, nyeri sendi dan otot, serta bintik-bintik merah terutama di badan dan tangan, meski gejalanya mirip dengan Demam Berdarah Dengue, pada Chikungunya tidak ada perdarahan hebat, renjatan ( Schok ) maupun kematian. Masa inkubasi : dua sampai empat hari, sementara Manifestasinya tiga sampai sepuluh hari. Virus ini tidak ada vaksin maupun obat khususnya, dan bisa hilang sendiri, namun, rasa nyeri masih tertinggal selama berhari-hari sampai berbulan-bulan.
Chikungunya
Beberapa minggu sebelum kasus SARS merebak, masyarakat Indonesia direpotkan dengan kasus chikungunya. Virus penyebabnya tergolong dalam Famili Togaviridae, yang belum diketahui pola masuknya ke Indonesia.
Sekitar 200-300 tahun lalu virus chikungunya (CHIK) merupakan virus pada hewan primata di tengah hutan atau savana di Afrika. Satwa primata yang dinilai sebagai pelestari virus adalah bangsa baboon (Papio sp), Cercopithecus sp. Siklus di hutan (sylvatic cycle) di antara satwa primata dilakukan oleh nyamuk Aedes sp (Ae africanus, Aeluteocephalus, Ae opok, Ae. furciper, Ae taylori, Ae cordelierri). Pembuktian ilmiah yang meliputi isolasi dan identifikasi virus baru berhasil dilakukan ketika terjadi wabah di Tanzania 1952-1953. Baik virus maupun penyakitnya kemudian diberi nama sesuai bahasa setempat (Swahili), berdasarkan gejala pada penderita. Maka hadirlah chikungunya yang berarti (posisi tubuh) meliuk atau melengkung (that which contorts or bends up). Setelah beberapa lama, perangai virus chikungunya yang semula bersiklus dari satwa primata-nyamuk-satwa primata, dapat pula bersiklus manusia-nyamuk-manusia. Tidak semua virus asal hewan dapat berubah siklusnya seperti itu. Di daerah permukiman (urban cycle), siklus virus chikungunya dibantu oleh nyamuk Ae aegypti. Beberapa negara di Afrika yang dilaporkan telah terserang virus chikungunya adalah Zimbabwe, Kongo, Burundi, Angola, Gabon, Guinea Bissau, Kenya, Uganda, Nigeria, Senegal, Central Afrika, dan Bostwana. Sesudah Afrika, virus chikungunya dilaporkan di Bangkok (1958), Kamboja, Vietnam, India dan Sri Lanka (1964), Filipina dan Indonesia (1973). Chikungunya pernah dilaporkan menyerang tiga korp sukarelawan perdamaian Amerika (US Peace Corp Volunteers) yang bertugas di Filipina, 1968. Tidak diketahui pasti bagaimana virus tersebut menyebar antarnegara. Mengingat penyebaran virus antarnegara relatif pelan, kemungkinan penyebaran ini terjadi seiring dengan perpindahan nyamuk.
Hasil penelitian terhadap epidemiologi penyakit chikungunya di Bangkok (Thailand) dan Vellore, Madras (India) menunjukkan bahwa terjadi gelombang epidemi dalam interval 30 tahun. Satu gelombang epidemi umumnya berlangsung beberapa bulan, kemudian menurun dan bersifat ringan sehingga sering tidak termonitor. Gelombang epidemi berkaitan dengan populasi vektor (nyamuk penular) dan status kekebalan penduduk. Pengujian darah (serologik) penyakit chikungunya sering tidak mudah karena serum chikungunya mempunyai reaksi silang dengan virus lain dalam satu famili. Untuk memperoleh diagnosis akurat perlu beberapa uji serologik antara lain uji hambatan aglutinasi (HI), serum netralisasi, dan IgM capture ELISA.

Musim penghujan sudah tiba. Dan nyamuk2 mulai bekembang biak secara pesat. Kejadian Luar Biasa (KLB) pun semakin banyak ditemui di berbagai daerah. Penetapan KLB berdasarkan undang-undang Wabah no 4 tahun 1984 adalah apabila terjadi peningkatan angka insiden (Incidence Risk) dua kali atau lebih daripada periode sebelumnya.
Kalau bicara masalah nyamuk tentu Chikungunya dan Demam berdarah merupakan penyakit yang sampai saat ini ditakuti. Chikungunya dan Demam berdarah disebabkan oleh nyamuk yang sama, Aedes aegypti. Hanya saja virus yang dibawanya berbeda, dan Chikungunya tidak mematikan.
Mari kita kenali demam chikungunya serta demam berdarah lebih lanjut.
Chikungunya
Penyakit chikungunya disebabkan oleh sejenis virus yang disebut virus chikungunya.Virus Chikungunya ini masuk keluarga Togaviridae, genus alphavirus.
Gejala-Gejala Demam Chikungunya :
1. Demam tinggi (39C)
2. Nyeri pada persendian (gejala khas demam chikungunya)
3. Tidak nafsu makan
4. Lemah
5. Mual
6. Sakit kepala
7. Timbul ruam merah seperti pada gejala demam berdarah
8. Fotophobia ringan
Penangganan demam chikungunya:
1. Minum obat penurun demam (paracetamol)
2. Melapor kepada petugas kesehatan yang terdekat jika panas tidak sembuh selama lebih dari 2 hari setelah pemberian obat
3. Minum obat pengurang rasa sakit (aspirin)
4. Jika menderita nyeri persendian yang hebat sebaiknya tidak bekerja terlalu keras terlebih dahulu karena dapat memperparah nyeri sendi tersebut
5. Meningkatkan daya tahan tubuh, yaitu dengan cara : Mengkonsumsi makanan yang bergizi tinggi, minum sari buah-buahan segar, serta istirahat yang cukup
Demam Berdarah
Penyakit Demam Berdarah atau Dengue Hemorrhagic Fever (DHF) ialah penyakit yang disebabkan oleh virus dengue yang ditularkan melalui gigitan nyamuk Aedes aegypti dan Aedes albopictus. Kedua jenis nyamuk ini terdapat hampir di seluruh pelosok Indonesia, kecuali di tempat-tempat ketinggian lebih dari 1000 meter di atas permukaan air laut. Namun menurut kepala Puskesmas Kepanjen Malang, dokter Hadi dikatakan belakangan kasus DBD bahkan ditemukan di puncak gunung Semeru. Jadi dimanapun kita harus tetap waspada.
Penyakit ini ditunjukkan melalui munculnya :
1. demam secara tiba-tiba 2-7 hari, disertai sakit kepala berat,
2. sakit pada sendi dan otot (myalgia dan arthralgia)
3. ruam; ruam demam berdarah mempunyai ciri-ciri merah terang, petekial dan biasanya mucul dulu pada bagian bawah badan - pada beberapa pasien, ia menyebar hingga menyelimuti hampir seluruh tubuh.
4. Pendarahan pada hidung dan gusi.
5. Selain itu, radang perut bisa juga muncul dengan kombinasi sakit di perut, rasa mual, muntah-muntah atau diare.

Karena seringnya terjadi perdarahan dan syok maka pada penyakit ini angka kematiannya cukup tinggi, oleh karena itu setiap Penderita yang diduga menderita Penyakit Demam Berdarah dalam tingkat yang manapun harus segera dibawa ke dokter atau Rumah Sakit, mengingat sewaktu-waktu dapat mengalami syok / kematian.
PENCEGAHAN
Kebanyakan dari masyarakat menganggap remeh program 3M (Mutup, Menguras, dan Menimbun) pemerintah. 3M sudah sering digembar-gemborkan pemerintah, namun tak kunjung diikuti oleh masyarakat, sehingga angka kejadian demam berdarah dan chikungunya tetaplah tinggi. Kalau ada kejadian baru bingung fogging, dll. Padahal perlu diketahui, Fogging hanya membunuh nyamuk dewasa. Sedangkan jentik2 nyamuk, calon nyamuk dewasa tetap hidup bebas dikamar mandi. Seperti pengalaman PKL kami kemarin di Kepanjen, Malang, ada satu RT yang terkena demam chikungunya, dan satu RT kena semua tanpa terkecuali, baik anak-anak maupun dewasa. Dan mereka baru bingung setelah terjangkit chikungunya. Nasi sudah terlanjur jadi bubur. Sudah terkena baru bingung. Semoga hal tersebut dapat memberikan pelajaran, sehingga kita tidak seperti mereka.
Jadi, kunci utama pencegahan Demam Berdarah dan Chikungunya adalah 3M.
3M versi Baru
3M pemerintah lalu dikembangkan menjadi 3M Plus. Yaitu dengan melakukan beberapa plus seperti memelihara ikan pemakan jentik, menabur larvasida, menggunakan kelambu pada waktu tidur, memasang kasa, menyemprot dengan insektisida, menggunakan repellent, memasang obat nyamuk, memeriksa jentik berkala, dll sesuai dengan kondisi setempat.
Terkait adanya konferensi perubahan iklim di Bali, tampaknya 3M perlu diubah. Karena salah satu M nya adalah menimbun barang-barang yang dapat menyebabkan air tergenang seperti kaleng bekas, plastik, dll. Dan ini merupakan pencemaran lingkungan yang nyata. Karena barang-barang tersebut (plastik, dll) sulit terurai oleh tanah. Dan akan terurai setelah berpuluh-puluh tahun kemudian. Dan sudah sepatutnya pemerintah mulai merubah M yang ketiga (mengubur), menjadi mendaur ulang atau bisa juga dengan merombeng (menjual) barang-barang bekas.

2. Menutup tempat penampungan air
3. Menjual barang-barang bekas

Hujan Berkurang Waspadai DBD dan Chikungunya

Yogyakarta (ANTARA News) - Memasuki musim pancaroba hujan mulai jarang turun sehingga masyarakat diimbau waspada karena pada saat itu dikhawatirkan terjadi peningkatan serangan penyakit demam berdarah dengue (DBD) dan chikungunya.

"Pada saat hujan mulai jarang turun justru jentik nyamuk akan berkembang pesat, karena jentik yang berada di genangan air dapat bertahan hingga tumbuh menjadi nyamuk. Sedangkan pada saat frekuensi hujan tinggi biasanya jentik tersebut terbawa aliran air hujan sehingga tidak berkembang," kata Kepala Dinas Kesehatan Kota Yogyakarta, dr Choirul Anwar, Sabtu.

Menurut dia, frekuensi hujan yang jarang mengakibatkan genangan air yang dapat menjadi tempat untuk sarang nyamuk.

"Bahkan nyamuk dapat bertelur dan berkembang di sisa air hujan yang berada di daun-daun seperti daun pisang atau talas yang terdapat di halaman rumah atau kebun," katanya.

Sementara itu Kepala Bidang Pengendalian Penyakit dan Penyehatan Lingkungan Dinas Kesehatan Kota Yogyakarta, dr Sri Wulaningsih mengatakan hingga Maret 2008 setidaknya 222 warga Yogyakarta terserang penyakit Chikungunya.

"Jumlah ini diperkirakan masih akan terus bertambah, karena pada musim pancaroba ini perkembangan nyamuk sangat pesat," katanya.

Ia mengatakan, saat ini sejumlah kawasan yang sebagian warganya terserang penyakit Chikungunya di antaranya Baciro, Warungboto, Prawirodirjan, Gedongkiwo, Kadipaten.

"Untuk memberantas penyakit chikungunya perlu ada kesadaran dari masyarakat untuk selalu menjaga kebersihan lingkungan. Upaya pemerintah tidak akan bisa berjalan maksimal tanpa dukungan dari masyarakat," katanya.

Delirium
DEFINISI
Delirium adalah keadaan yang yang bersifat sementara dan biasanya terjadi secara mendadak, dimana penderita mengalami penurunan kemampuan dalam memusatkan perhatiannya dan menjadi linglung, mengalami disorientasi dan tidak mampu berfikir secara jernih.
PENYEBAB
Delirium merupakan suatu keadaan mental yang abnormal, bukan suatu penyakit; dengan sejumlah gejala yang menunjukkan penurunan fungsi mental.

Berbagai keadaan atau penyakit (mulai dari dehidrasi ringan sampai keracunan obat atau infeksi yang bisa berakibat fatal), bisa menyebabkan delirium.

Keadaan ini paling sering terjadi pada usia lanjut dan penderita yang otaknya telah mengalami gangguan, termasuk orang yang sakit berat, orang yang mengkonsumsi obat yang menyebabkan perubahan fikiran atau perilaku dan orang yang mengalami demensia.

Penyebab delirium:
• Alkohol, obat-obatan dan bahan beracun
• Efek toksik dari pengobatan
• Kadar elektrolit, garam dan mineral (misalnya kalsium, natrium atau magnesium) yang tidak normal akibat pengobatan, dehidrasi atau penyakit tertentu
• Infeksi akut disertai demam
• Hidrosefalus bertekanan normal, yaitu suatu keadaan dimana cairan yang membantali otak tidak diserap sebagaimana mestinya dan menekan otak
• Hematoma subdural, yaitu pengumpulan darah di bawah tengkorak yang dapat menekan otak.
• Meningitis, ensefalitis, sifilis (penyakit infeksi yang menyerang otak)
• Kekurangan tiamin dan vitamin B12
• Hipotiroidisme maupun hipotiroidisme
• Tumor otak (beberapa diantaranya kadang menyebabkan linglung dan gangguan ingatan)
• Patah tulang panggul dan tulang-tulang panjang
• Fungsi jantung atau paru-paru yang buruk dan menyebabkan rendahnya kadar oksigen atau tingginya kadar karbon dioksida di dalam darah
• Stroke.
GEJALA
Delirium dapat diawali dengan berbagai gejala, dan kasus yang ringan mungkin sulit untuk dikenali.
Tingkah laku seseorang yang mengalami delirium bervariasi, tetapi kira-kira sama seperti orang yang sedang mengalami mabuk berat.

Ciri utama dari delirium adalah tidak mampu memusatkan perhatian.
Penderita tidak dapat berkonsentrasi, sehingga mereka memiliki kesulitan dalam mengolah informasi yang baru dan tidak dapat mengingat peristiwa yang baru saja terjadi.

Hampir semua penderita mengalami disorientasi waktu dan bingung dengan tempat dimana mereka berada.
Fikiran mereka kacau, mengigau dan terjadi inkoherensia.

Pada kasus yang berat, penderita tidak mengetahui diri mereka sendiri.
Beberapa penderita mengalami paranoia dan delusi (percaya bahwa sedang terjadi hal-hal yang aneh).

Respon penderita terhadap kesulitan yang dihadapinya berbeda-beda; ada yang sangat tenang dan menarik diri, sedangkan yang lainnya menjadi hiperaktif dan mencoba melawan halusinasi maupun delusi yang dialaminya.

Jika penyebabnya adalah obat-obatan, maka sering terjadi perubahan perilaku.
Keracunan obat tidur menyebabkan penderita sangat pendiam dan menarik diri, sedangkan keracunan amfetamin menyebabkan penderita menjadi agresif dan hiperaktif.

Delirium bisa berlangsung selama berjam-jam, berhari-hari atau bahkan lebih lama lagi, tergantung kepada beratnya gejala dan lingkungan medis penderita.
Delirium sering bertambah parah pada malam hari (suatu fenomena yang dikenal sebagai matahari terbenam).
Pada akhirnya, penderita akan tidur gelisah dan bisa berkembang menjadi koma (tergantung kepada penyebabnya).

DIAGNOSA
Diagnosis ditegakkan berdasarkan gejala-gejalanya dan sesegera mungkin ditentukan penyebabnya.

Dilakukan pemeriksaan fisik lengkap dan dititikberatkan pada respon neurologis penderita.

Pemeriksan lainnya yang biasa dilakukan adalah pemeriksaan darah, rontgen dan pungsi lumbal.
PENGOBATAN
Pengobatan tergantung kepada penyebabnya;
- infeksi diatasi dengan antibiotik
- demam diatasi dengan obat penurun panas
- kelainan kadar garam dan mineral dalam darah diatasi dengan pengaturan kadar cairan dan garam dalam darah.

Untuk meringankan agitasi diberikan obat-obat benzodiazepin (misalnya diazepam, triazolam dan temazepam).

Obat anti-psikosa (misalnya haloperidol, tioridazin dan klorpromazin) biasanya diberikan hanya kepada penderita yang mengalami paranoid atau sangat ketakutan atau penderita yang tidak dapat ditenangkan dengan benzodiazepin.

Jika penyebabnya adalah alkohol, diberikan benzodiazepin sampai masa agitasi penderita hilang.
